Both Peter's maternal and paternal grandparents are alive, there no feud between the two side of the family and they no obstacle for any of them to come to visit. Yet, only one pair ever come to his birthdays.
Why So ?

The word pagan came from paganus , who mean peasant . Its was a way to significate than christianism was the religion of the elite and paganism the one of the savage worker class.
''Trickster shows us how we trick OURSELVES. Her rampant curiosity backfires, but, then, something NEW is discovered (though usually not what She expected)! This is where creativity comes from—experiment, do something different, maybe even something forbidden, and voila! A breakthrough occurs! Ha! Ha! We are released! The world is created anew! Do something backwards, break your own traditions, the barrier breaks; destroy the world as you know it, let the new in.'' Extract of the Dreamflesh article ''Path of The Sacred Clown''

Is this a riddle? The family is very inbred and the maternal and paternal grandparents are the same people?

Peter refuses to wash from one year top another and only one set of grandparents are anosmic (Peter may have inherited it, but they haven't found a doctor who will go close enough to test him). If Peter's parents divorced and they both married a parent-in-law, the word 'pair' wouldn't be appropriate. And they wouldn't stop being grandparents even if they were step-parents anyway. Silly Sir Henry .

Cultural reasons? There a certain religious sects who do not celebrate birthdays, or the birthday may be on a day that is believed to be inauspicious to travel. They may have some higher social obligation that compels their attendance.

Huh, I guess there is. Was my first conclusion, then I saw the post saying he lived with one set, and realized it was much more reasonable. With both angles covered, I didn't bother to post - can't think of a third option that makes sense. As a further twist, how about this continuation: All of Peter's Grandparents and both of Peter's parents are alive and attend his birthday party, but only 3 people show up. Explain.

or to add an antipodean colonial twist : All Peter's grandparents and parents, an aunt, an uncle and sibling are alive . He spends his birthday with all 3. 2 people take him down to the shearing shed.

Another possible option, if only one pair ever visits for his birthday maybe he visits the other pair on his birthday, possibly even taking it in turns to visit a different set each year. Therefore only one set ever needs to travel to visit him.
