The back piercing kind of reminds me of a particular scene in the movie "The Cell" (Jennifer Lopez, Vincent D'Onofrio, 2000).  Bad story, terrible, acting but  extremely well done special effects; very disturbing yet interesting visuals.  Shock is 50% of the aesthetic value of this.
